Because we are a group of individuals and ministries working together to bring about Revival in our land we ask that each one of us be personally responsible for their own safety and come prepared with their own medical coverage.  To ensure that each of us take personal responsibility for themselves it is important that they know the risk involved with any ministry.  We ask that you print out the form, read it carefully, answer all the  questions thoughtfully and prayerfully and mail it to us at:
